POLICIES

MEDICAL INSURANCE & FINANCIAL POLICY 

Please read the following information carefully.

If you have any questions regarding our Financial/Medical insurance policy, you may contact our billing department.

  • We verify eligibility at every appointment; however, the information we receive is very basic. For detailed information regarding your insurance benefits, please contact your insurance company directly.

  • ANY SERVICE RENDERED WILL BE THE RESPONSIBILITY OF THE PATIENT IF THE INSURANCE DOES NOT COVER THE SERVICE.

  • All payments are due at the time of service. Including any co-pay, co-insurance, deductibles or self-pay charges incurred. The person who brings the child in for the visit is responsible  for the payment at check-in.

  • When a patient has a deductible, our office policy is to collect $75.00 towards your deductible at time of visit. Once the claim is processed and posted it will result in a balance or a credit for that date of service.

  • We participate with many different insurance companies. In addition, each of these companies offers many different types of plans.

  • We verify on the date of service that there is active coverage on the policy and make every attempt to accurately collect co-pays, co-insurance & deductibles.

  • It is the parent/guardians responsibility to know and understand your insurance coverage and benefits. Any patient who is seen and fails to notify our office of any changes in their insurance that in turn deems our service as non-covered will be billed directly for the charges. Any questions concerning your coverage should be directed to your insurance company. Therefore, your balance is your responsibility. Should there be a dispute with your insurance company, our billing department will attempt to resolve it for you. During this time, the balance may be transferred to patient responsibility. 

  • Some insurance plans require a Well Check once every 365 days, and some require an annual Well Check. It is your responsibility to find this information out from your insurance company and schedule the Well Visit according to your plans guidelines.

  • If a PCP is required for your insurance please make sure our Physician is listed as PCP; otherwise you will be financially responsible for services rendered. This statement applies to some commercial insurance companies and all patients covered under a Bayou Health / Medicaid Policy.

  • It is your responsibility to know if your insurance requires a Referral for a Specialist or Prior Authorization for any medicine and out-patient hospital services ( blood work, genetic testing, injections, X-Rays and any other radiological testing.)

Please DO NOT have services performed until Referral or Prior Authorization is completed and approved by insurance.

APPOINTMENT POLICY

If you are unable to keep your scheduled appointment, please notify us at least 24 hours in advance, so we can accommodate our other patients.

You may also reschedule your appointment at this time.

Our No Show Policy is as follows:

  • A 24 hour notice is required.

  • After the FIRST no show appointment you will receive a letter to remind you of the missed appointment & to reschedule your appointment.

  • After the SECOND no show, you (not your insurance company) will be charged $25 for the time slot we were unable to fill due to your absence.

  • On the THIRD no show, it will be at the discretion of the physician as to whether a discharge letter will be sent disengaging you from the practice & giving you 30 days to enroll with a new physician.

VACCINE POLICY

All patients in the practice are required to receive:

  • A minimum of DTaP, Hib, Polio & pneumococcal vaccines by 3 months of age.

  • All AAP-recommended immunizations by 2 years of age.

  • Meningococcal vaccine / Booster doses of Tdap and. Varicella vaccines by 12 years of age. 

If you choose to not vaccinate your child despite all of our efforts, we will ask you to find another healthcare provider who shares your views.  We do not keep a list of such providers, nor would we recommend any such physician. Please recognize, by not vaccinating your child, you are putting your child and the children your child is exposed to at unnecessary risk for
life-threatening illness, disability and even death.
